How to set RCP Packager window icon?

I've successfully created an RCP Packager installer and it seems to work ok for the small project I'm working on. The icon for the .exe is the standard windows icon, but once the installer is running, the window title icon is just a blank white box. How do I set the icon for the window title? I've looked, but it's possible I missed something obvious.

It would also be nice to specify my own banner image.

There's no easy way to incorporate this with the installer. There might be some 3rd party standard Windows icon tools to manually change the icon of your installer.
