Actions in Application Windows don't work with 1.2.2beta

Hi
This is a great product, but sometimes, I have some dumps.

I have a problem with the new 1.2.2 beta version (pro) which I used, because I had the same problem with 1.2.1:
When I create a new JFace ApplicationWindow with the wizard, change to design, action, then I might add one menu point and one or perhaps two actions without any problem.
But when I add the third action, I get the following error log and no action is added.
As you can see, I'm using Windows.
As I said before, the same happened with 1.2.1
Did you ever notice this?
By the way, there is no documentation about using actions, I found it only here on this forum.

The 1.2.2 beta has been updated and includes fixes for the above.
